2020-02-03 · Delirium is a common postoperative complication that occurs in 5 to 52% of older adult patients after noncardiac surgery [1, 2].Delirium is characterized by disturbance in attention, awareness, and cognition that develops acutely and fluctuates frequently throughout the course of the condition []. Postoperative delirium is a main complication after extensive surgical procedures. It often arises within a few days after surgery also gets a quick recovery. The 2010 National Institute for Health and Care Excellence (NICE) delirium guidelines described risk factors: age over 65 years, and pre-existing cognitive impairment, and current hip fracture or serious illness ( 2 ). Delirium is a common postoperative neurocognitive complication in the older population and can lead to significant morbidity and mortality, as well as cognitive and functional impairment. Hypoactive delirium is characterised by drowsiness and inactivity, and expert opinions suggest that it is more likely to be missed and can lead to more complications. Background Delirium is common after cardiac surgery and may be associated with long-term changes in cognitive function.

Postoperative delirium, which may be complicated by these conditions, bears a temporal and etiological relationship to surgery, usually developing between postoperative days 2 and 7. Figure 1. Etiologies and a Pathophysiological Model of Delirium a Postoperative delirium is a serious problem that is associated with prolonged hospital stay and increased mortality. The authors review the pathophysiology of post-operative delirium, identifying risk factors and poten-tially modifiable factors.

Postoperative delirium also increases the individual's risk of other complications whilst in hospital, including falls and aspiration pneumonia (Robinson & Eiseman 2008). Postoperative delirium is a common complication and exerts an enormous burden on patients, their families, hospitals, and public resources. To alleviate this burden, this article aimed to review general features and the latest evidence-based knowledge of delirium with a focus on postoperative delirium. Post-op delirium (also known as “POD”) is a form of delirium seen in patients who have undergone anesthesia for a surgical procedure. The prevalence of delirium in postoperative patients ranges from 10% to 51% (5). Cardiac surgery in general poses an even greater risk (6). The incidence of delirium in elderly postoperative patients is 15% to 53% and as high as 80% in elderly patients who require intensive care (7).

Outcome and quality of life in patients with postoperative delirium during an ICU stay following major surgery. If we say that burst suppression (an EEG pattern) mediates the effects of the other factors on postoperative delirium, we are saying that these other factors would not be able to cause (as much) delirium, unless they specifically interacted with the anesthesia and surgery in such a way as to produce a pathologic intraoperative brain state that was manifest as a burst suppression pattern in the EEG. Postoperative delirium is a common, life-threatening problem in older adults and is recognized as the most common postoperative complication in this age group. 1, 2ET Delirium occurs after surgery in 5% (in Postoperative delirium increased the risk of postoperative cognitive dysfunction at 1 month postoperatively but there was no association between postoperative delirium and cognitive dysfunction at 2 and 6 months after major noncardiac surgery Postoperative delirium and longer-term postoperative cognitive dysfunction may be different disorders Postoperative delirium may be difficult to diagnose due to many risk factors and its nonspecific pathophysiology. This situation makes the diagnosis and treatment of the syndrome difficult.
